Bring your photo ID, insurance card, and a list of all medications to your appointment.

If you are unable to keep your appointment, notify us at least 24 hours in advance to avoid a possible $50 Appointment Violation Fee.

Our office is in the 600 Building of Central Professional Park. Look for a sidewalk sign that says "600 Building". 

Type Suite "612" into the address if you are using your GPS: 2609 N DUKE ST SUITE 612, DURHAM, NC 27704.

All new patients should arrive 30 minutes before their appointment to complete the registration process.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with our office. We look forward to your first visit.

Handicap Accessible

We have convenient parking near our office entrance. Our treatment rooms are also located a short distance from our lobby. We have a handicap accessible exam chair. Our staff can also provide some aspects of assistance. However, their abilities may be limited based on your needs. If you have limited mobility, consider bringing your wheelchair, walker, or a caregiver to assist you during your appointment. 

How to Make the Most Out of Your Appointment by Being Prepared

1. Bring your up-to-date insurance card(s).

2. Bring a photo ID.

3. Confirm that the office has your current phone number, address, and pharmacy location.

4. Have a written or typed list of your medications.  Include the name, strength, and frequency.

5. Know your past medical problems and past surgeries.

6. Arrive early for your appointment. You will be more present for your appointment if you aren’t feeling rushed.

7. Bring a family member or friend to your appointment if you have trouble remembering information or getting around easily.

8. Have a list of prepared questions. Rehearse how to communicate your concerns in a clear and concise manner.

9. Tell the doctor if you have been placed on a new medication. Know why you are taking it. Know how to properly take it.

10. Keep a written list of doctor’s appointments. Include the name of the doctor, phone number, and address.
